- Both parties must have a valid passport
- Original birth certificate of each person
- Statuatory of Declaration form must be notarized by
a lawyer/solicitor (a sample can be provided by the hotel
if requested). Both parties must have one form each notarized
- Statuatory of Declaration from must be translated
into Spanish. Statuatory documents must be
legalized by the Dominican Consulate. You
will need to send both the original form in
English and 2 of the translated forms. The
cost is $50 US per document. Only the translated
copies are legalized and stamped, so these
are the ones you have to pay for. The English
copies are for their verification purposes.
